Interim of Oakland County has been providing home care services to customers in Oakland and surrounding counties for the last twelve (12) years. Interim of Oakland County (IOC) provides nurses, therapists, and aides to private homes and schools, as well as medical offices
Our agency has been CHAP (Community Health Accreditation Program) accredited since 2010. We employee individuals with national certifications; AHA BLS instructor, AHIMA certified coder. We also have a CRNI (Infusion Specialty) nurse on staff to meet our training and delivery needs. Our commitment is to the highest professional standards and customer.
Our office team consists of 5 customer service representatives, a receptionist, and 2 quality assurance specialist. A dedicated 24/7 phone is answered by a CSR team member to assist with any afterhours emergencies.
We believe it is important to provide the right level of services, so we perform a comprehensive assessment by a highly qualified professional that helps you make informed decisions. We also believe communication is important so, with permission, we provide a written report of the client’s skilled and /or unskilled needs. This report addresses the following:
Home Safety;
Understanding of medical conditions and how to manage them;
Functional status – ability to handle the activities of daily living;
Medications – ability to take them correctly – time and dosage;
Nutritional habits and status;
Mental outlook
We develop a customized service plan to support the level of care necessary for each individual client. It’s often a matter of little help and usually not about acute care or around the clock needs. It is about helping with the little things that make every day safe. Our individualized plans are meant to ensure our clients enjoy independence and quality of life.

Ongoing assessments and reports are available periodically or after significant events such as hospitalization, emergency room visits, falls or some other event or behavioral change.
All of our clients receive a visit from a Registered Nurse every 60 days. As time progresses our clients needs may change. This visit helps insure our service plan is custom tailored to our clients needs.
